The Wheel Of Misfortune card is the pessimistic-sounding shadow/invert of the Major Arcana's symbol of fate, chance, destiny, and luck.
While seemingly negative and opposite from tarot's standard "Wheel of Fortune" card, one could argue that philosophically they mean the same thing.
In the same way you could arbitrarily call any point on a circle, "the beginning" or "the end," you can mistakingly view fortune and misfortune as unrelated, untethered concepts. In reality, fortune and misfortune arrive together in single file (or hand-in-hand). Time becomes the hyper-flexible alignment system by which we ultimately judge fate.
